 note = {【背景】臓器移植法が施行されたこの10年で62件の脳死移植が行われた。わが国の移植医療の足踏みは,何が原因か,どこに問題があるのかの明確化が問われている。【対象と方法】日米の移植医療を比較し,現実的な移植医療環境のわが国への応用を目指し,S.W.O.T. (strength, weakness, opportunity, threat)と呼ばれる医療経済学で用いられる枠組みに照らして,社会的背景,医療環境,宗教を含む国民性等を分析した。それをもとに,術前の集団インフォームド・コンセントを試み,国内移植施設において施行した。その結果をもって,国内腎移植施設〔40施設〕・肝移植施設〔24施設〕にて患者への説明および教育指導,携わる職種等の実態調査を行った。【結果】集団インフォームド・コンセントに関する施行後の患者に対するインタビューおよびアンケート調査では,概ね高い評価を得た。また,移植施設調査では,1.レシピエントに比してドナーに対するかかわりが少なかった。2.医師が多くの役割を担っていた。3.腎移植と肝移植の差異が明確になった。【考察】わが国の社会的背景は,米国と大きく状況を異にするものであったが,応用した集団インフォームド・コンセントは,患者から高い評価を得た。これは,患者間連携の有用性を示していると思われる。【結論】今後は移植施設のみならず,透析施設における医師の情報提供や説明に関する調査によって,さらなる課題が明らかになると思われる。, Background: There have been only 62 brain-dead donors who had undergone transplant process under the Organ Transplant Law in Japan for these ten years. Currently, the transplant care in our country has been at a standstill because of the insufficient supply of transplantable organs. Thus, it is urgently necessary for our country to resolve these problems at present. Objective: Verification of the effectiveness of pre-operative group informed consent (GIC). Method: To accomplish the application of practicable transplant medical conditions for our country in contrast with the United States, we analyzed social background, medical conditions, the national character including its religion of two countries using the framework called 'SWOT' (strength, weakness, opportunity, thread ) for medical economics. Based on the results of the analyses, we obtained preoperative GIC from patients and their relatives, gave further educational guidance to them, and investigated the actual conditions of relative medical staffs in the 40 domestic transplant institutions of kidney, 24 of liver. Results: Almost patient appreciated GIC. Investigation of the transplant institutions revealed: (1) Care for donor and their relatives were less sufficient than that for recipients, (2) Doctors took most of roles rather than coordinators, (3) The difference between kidney and liver transplant became clearer. Discussion: The reason of high evaluation for GIC might imply importance and effectiveness of close cooperation among patients.
